Masters Degree in Nursing and a Certificate as a Certified Family Nurse Practitioner (FNP) with a current active unrestricted license to practice as a registered nurse and FNP in the state of Maryland. Required experience of 2-5 years as a FNP with relevant clinical experience within the past 2 years sufficient to demonstrate current clinical competency for the setting of a family practitioner. The Home-Visit Nurse Practitioner is responsible, as part of the patient care team, for the overall patient care management process. An advocate for the patients, the Nurse Practitioner will provide total care for the patients assigned in collaboration with the physician of record and patient care team.
