<p>The Sequoia Teen Wellness Center, committed to providing health care services to youth (ages 12-21) while respecting their privacy, is currently seeking a Nurse Practitioner. The services offered at the Sequoia Teen Wellness Center include primary care, physicals/exams, immunizations, reproductive health services, health education, mental health services and support groups. Our Wellness Center is conveniently located in Redwood City, across from Sequoia Station and next to the Sequoia High School campus, and provides services regardless of ability to pay, citizenship or school enrollment.</p>
<p>This is an exciting opportunity for an NP who enjoys working with adolescents in a small, dedicated practice serving Redwood City. The clinic is a unique collaboration between the school district and San Mateo Medical Center. The NP serves in a semi-autonomous role as a medical provider and the clinic team consists of a physician, two nurse practitioners, an LVN, a medical assistant, mental health practitioners and clerical staff.</p>
<p>Qualifications include possession of a CA RN license and Nurse Practitioner certification in primary health care for patients of all ages, as well as fluent language skills in both English and Spanish.</p>

<p>The Esperanza Center, a program of Catholic Charities provides health care, community services, educational and legal services for the underserved and uninsured immigrant population. Esperanza Center’s Health Services (ECHS) has adopted the VIM (Volunteer in Medicine) model, engaging physicians, nurses and other health care professionals to provide quality health care free of charge. Presently, we offer primary adult and pediatric care, well-women, ophthalmology, neurology and dental services in a community oriented environment. We partner with local hospitals, including Johns Hopkins, St. Agnes, and University of Maryland Dental and other area service providers to insure accessible, quality care.</p>
<p>We provide a work environment that offers reasonable hours, great benefits, professional liability coverage, competitive salary and a caring and committed staff of professionals and volunteers with a passion for serving those in need.</p>
<p>The Family Nurse Practitioner will be responsible for overseeing all clinical aspects of our primary care services, as well as providing direct health care services to the clients at the Esperanza Center. Duties will include the assessment, diagnosis, and treatment of patients of all ages with acute and chronic illness, ordering of laboratory and other diagnostic tests, patient referrals and the prescription of medication. The Nurse Practitioner will collaborate with medical and other program staff and participate in preventative health programs to facilitate the optimal health of clients. Collaborates on the development and management of clinical policy and procedure manuals.</p>
<p>The Nurse Practitioner will maintain clinical records on all patients and collaborate with Esperanza Health Manager in the development of Clinical Manuals Policies and Procedures, standards of care and quality assurance as needed in the transition to a Volunteer in Medicine Clinic. Serves as a member of Quality Assurance, Credentialing and Peer Review Committees as needed and appropriate.</p>
<p>Masters Degree in Nursing and a Certificate as a Certified Family Nurse Practitioner (FNP) with a current active unrestricted license to practice as a registered nurse and FNP in the state of Maryland. Required experience of 2-5 years as a FNP with relevant clinical experience within the past 2 years sufficient to demonstrate current clinical competency for the setting of a family practitioner. Current adult and pediatric CPR Health Care Provider required. Effective communication and interpersonal skills required. Ability to communicate medical information effectively in English required. Proficiency in Spanish required. Candidates must provide 2 letters of recommendation written within the last 2 years attesting to clinical skills, with one letter from a past supervisor. Must complete full Credentialing to include: DEA, CDS, NPI and license verification.</p>

<p>Duties:<br />
<br />The Nurse Practitioner will be responsible for the total care of the assigned patient population in collaboration with the Patient Care Team (PCT), and/or Network Clinical Director and community based physicians/providers.<br />
<br />This position is accountable for identifying and developing innovative actions to meet the needs of INSPIRIS patients from both the health care and psychosocial / socioeconomic dimensions of care; as well as taking action for provision of services to meet the patient needs.<br />
<br />The Nurse Practitioner will be responsible as part of the PCT for patient assessments, risk management plan development, maintenance care, acute interventions within scope, procedures such as IV access and wound care (not all inclusive), documentation and patient education.<br />
<br />The Nurse Practitioner will share in the responsibilities and care of patients located in understaffed and rural/remote areas of the network and adjacent network(s) as determined through team planning.<br />
<br />The Nurse Practitioner will ensure that all pertinent and active medical conditions are documented in the medical record in a manner compliant with CMS, INSPIRIS and client requirements.<br />
<br />As part of a call schedule, the Nurse Practitioner will provide 24/7 patient access.</p>
<p>Active un-encumbered license to practice nursing in the state where the position is located.<br />
<br />Nurse Practitioner Certification from either the ANCC or AANP.<br />
<br />Geriatric or adult medicine specialty a plus.<br />
<br />Minimum of 1 year experience.<br />
<br />Must be bilingual in English and Spanish (verbal and written)<br />
<br />Must be able to effectively communicate with elderly and chronically ill patients and families.<br />
<br />Understanding of Medicare, Medicaid and Health Plan benefit structures beneficial.<br />
<br />Comfortable working independently in a home-visit setting.<br />
<br />Willingness to perform procedures for self and other provider&#8217;s patients.<br />
<br />Basic knowledge of computers and cell phones.<br />
<br />Familiarity with ICD-9 and CPT-4 coding.<br />
<br />BCLS<br />
<br />Excellent organizational skills with the ability to multi-task.</p>

<p>The Nurse Practitioner in Urology is an advanced practice nurse who provides direct nursing care to the pediatric urology patients, emphasizing health promotion and disease prevention, as well as the diagnosis and management of acute and chronic diseases in both the inpatient and outpatient settings. Specific clinical functions will include participating in outpatient procedures such as urodynamic studies and biofeedback for voiding dysfunction/neurogenic bladder.  Services include but are not limited to ordering, conducting and interpreting appropriate diagnostic and laboratory tests, prescription of pharmacologic agents and treatments and non-pharmacologic therapies in a cost efficient manner. Works autonomously as well as in collaboration with a variety of individuals to diagnose and manage clients’ health care problems. Serve as health care resources, interdisciplinary consultants and patient advocates. Demonstrates commitment to a patient and family focused model of care. </p>
<p>This position is an 8 or 10 hour shift, no nights, and no Holidays. </p>
<p>Qualifications: Masters Degree in Nursing required. Three years pediatric experience required. One year of urology experience preferred. Bilingual English/Spanish required.  </p>
<p>License, Registration or Certification Required: Graduate of an accredited Nurse Practitioner Program approved by the California Board of Registered Nursing for “hold out as a Nurse Practitioner” as defined in Section 1482, Nurse Practice Act, Business and Professional Code. Current California Registered Nurse License and current certification as a Nurse Practitioner. Current BCLS and PALS certification required. National certification as appropriate &#8211; preferred. Eligible for Furnishing License &#8211; required. </p>

The health center will provide comprehensive primary and behavioral care services, serving 3,000 patients each year.  The health center managed by nurse practitioners will provide much needed care to a vulnerable population that faces significant health disparities. </p>
<p>RESPONSIBILITIES:<br />
<br />•	Responsible for effective leadership and management strategies that maximize effectiveness, efficiency and productivity in the provision of primary care, including continuous quality improvement and protocol development<br />
<br />•	Orient, supervise and evaluate primary care professionals and support staff<br />
<br />•	Coordinate and provide primary care services to target population<br />
<br />•	Coordinate and maintain staff scheduling and appropriate staffing patterns for primary care services at center<br />
<br />•	Lead monthly (or as needed) management staff meetings<br />
<br />•	Identify clinical staff continuing education needs<br />
<br />•	Work within all state and federal laws, regulations and collaborating physician agreements.<br />
<br />•	Advance role of advanced practice nurses in underserved areas through advocacy and other professional activities.<br />

<br />•	Insure open communication between PHMC and the practice<br />
<br />•	Represent health center as necessary to community organizations<br />
<br />•	Maintain positive relationships with funding sources and community organizations<br />
<br />•	Perform other tasks as directed.<br />
<br />•	Participate in regularly scheduled administrative/management meetings<br />
<br />•	Assist in writing proposals, reports and program evaluations as necessary<br />
<br />•	Collect staff data as necessary for reports to other agencies.</p>
