<p>JOB SUMMARY</p>
<p>The RN Case Manager in Ambulatory is responsible for actively managing and advocating for patients currently enrolled in the Community Special Services Program. The Case Manager will be responsible for having comprehensive knowledge of the patient&#8217;s medical status. The Case Manager will consider physical, psychosocial and financial assessment information in planning for, coordinating and implementing a patient specific plan of care. Case management will include educating patients about medication, encouraging adherence, promoting risk reduction and healthy lifestyle choices, and follow up with patients to see that medication is taken, appointments are made and labs are completed. The Case Manager will work with providers and other multidisciplinary team members, utilizing a proactive approach, to ensure that medical care is provided based on current standards of care.</p>
<p>•Provides direct patient care, evaluates outcomes, consults with other healthcare team members as required and adjusts nursing care processes as indicated to ensure optimal patient care.</p>
<p>•Medical and surgical asepsis is carried out during treatments and special procedures.</p>
<p>•Consults other departments, as appropriate, to provide for an interdisciplinary approach to the patient’s needs.</p>
<p>•Maintains a good working relationship within the department and with other departments.</p>
<p>•Demonstrates the ability to be flexible, organized and function under stressful situations.</p>
<p>•Provides ongoing patient education</p>
<p>•Identifies patient’s needs for additional medical, psychosocial and financial recourses</p>
<p>•Coordinates patient’s treatment with other organizations</p>
<p>•Utilizes knowledge of preventive care guidelines and works with all primary care providers to ensure that the appropriate screening exams and vaccinations are current.</p>
<p>•Excellent oral and written communication skills</p>
<p>•Good team player</p>
<p>MINIMUM REQUIRED: Graduation from an accredited School of Nursing or successful completion of sufficient units in Nursing to qualify for California licensure.</p>
<p>PREFERRED: 3+ years disease specific experience (e.g. HIV/AIDS). Medical/Surgical or critical care experience. Bilingual in Spanish preferred.</p>

<p>The Clinical Appeals Process focuses on the review and analysis of lower level Medicare denial rationales and provides appropriate medical necessity appeal services. The Appeals Nurse will review Medicare contractor denial rationales and case-specific medical records for compliance, accuracy and completeness in accordance with department processes, followed by writing and editing appeal letters for client hospitals, subject to Physician review.</p>
<p>Job Responsibilities</p>
<p>• Review Medicare contractor response letters in comparison to client medical records</p>
<p>• Communicate with clients regarding missing or insufficient medical documentation</p>
<p>• Make referrals to Regulatory Team or Medical Director</p>
<p>• Review medical documentation for adherence to Medicare guidelines relating to inpatient services (or other Medicare issues) and draft appropriate appeal letters based upon professional clinical opinion as to the medical necessity of the services provided</p>
<p>• Complete data entry in the company&#8217;s proprietary technology</p>
<p>• Meet daily minimum expectation for assigned tasks</p>
<p>• Perform other related duties as required</p>
<p>Hours</p>
<p>• Day hours Monday through Friday, with occasional evening and weekend hours as needed.</p>

<p>The College: The College of Health and Human Services is located in a new 195,000 square foot building with state-of-the-art teaching technologies and spacious research laboratories. The college enrolls over 2,200 students, with more than a third of those at the graduate level. In addition to WMU Bronson School of Nursing, the college includes programs in blindness and low vision studies, occupational therapy, physician assistant, social work, speech pathology and audiology, holistic health care, alcohol and drug abuse, interdisciplinary health services, and interdisciplinary health sciences. The college has received a number of grants from the National Institutes of Health (NIH), the Health Resources and Services Administration (HRSA), and the Department of Education. The college also operates the Unified Clinics, a multidisciplinary service/training clinic.</p>
<p>School: The WMU Bronson School of Nursing is a vibrant department that focuses on the scholarship of teaching, research, and service. In addition to the Director of the Bronson School of Nursing, over 23 full-time and 15 part-time faculty provide instruction and experiences for prelicensure (BSN), Registered Nurse to Bachelor of Science in Nursing (RN-BSN), and Master of Science in Nursing (MSN) students. The total enrollment includes approximately 390 undergraduate students and a cohort of MSN students. Currently, the NCLEX pass rate ranges from 95.2-98.8 over the past 2 years. The WMU Bronson School of Nursing is granted full approval by the Michigan Board of Nursing. The undergraduate program was recently re-accredited by the Commission on Collegiate Nursing Education for 10 years and the master’s program was accredited for 5 years. The WMU Bronson School of Nursing is one of the few nursing schools to receive endorsement from the American Holistic Nursing Certification Corporation.</p>

<p>The University of Virginia Health System in Charlottesville is currently seeking an RN Clinician 2 for its Operating Room.</p>
<p>Responsibilities:</p>
<p>• Consistently provide effective direct care, as part of the interdisciplinary team, to a variety of complex patients</p>
<p>• Seek as well as provide feedback for improved clinical practice</p>
<p>• Assume a beginning leadership role but seek mentoring in this process</p>
<p>Qualifications:</p>
<p>• Graduate of an accredited nursing program</p>
<p>• 1-2 years’ experience</p>
<p>• Must be a registered nurse and currently licensed to practice in the State of Virginia</p>
<p>• American Heart Association (AHA) Health Care Provider BLS certification required</p>
<p>• Employment in this job is conditioned upon satisfactory completion of a drug test</p>

<p>San Mateo Medical Center’s Fair Oaks Children’s Clinic in Redwood City is currently seeking a Charge Nurse who is looking for an exciting opportunity to help develop our new pediatric outpatient clinic. The Charge Nurse, in collaboration with the Clinic Manager, will direct nursing care, coordinate resources for the clinic and perform administrative activities while providing leadership and support to staff. In addition, the incumbent will provide quality care to patients, participate as a member of the clinic management team in the development and implementation of clinic goals, and interface with the supervising physician and clinic manager for short-term problem solving.</p>
<p>The ideal candidate will be an RN with at least 2 years of pediatric outpatient nursing experience, at least 1 year of supervisory experience and a background providing education and updates to staff and scheduling for multiple shifts. Excellent communication skills are essential. Prefer knowledge of quality improvement processes and community health systems, background working with an underserved population, computer literacy, experience with electronic medical records and registries, as well as bilingual English/Spanish abilities. Current California RN license is required.</p>

<p>The Sequoia Teen Wellness Center, committed to providing health care services to youth (ages 12-21) while respecting their privacy, is currently seeking a Nurse Practitioner. The services offered at the Sequoia Teen Wellness Center include primary care, physicals/exams, immunizations, reproductive health services, health education, mental health services and support groups. Our Wellness Center is conveniently located in Redwood City, across from Sequoia Station and next to the Sequoia High School campus, and provides services regardless of ability to pay, citizenship or school enrollment.</p>
<p>This is an exciting opportunity for an NP who enjoys working with adolescents in a small, dedicated practice serving Redwood City. The clinic is a unique collaboration between the school district and San Mateo Medical Center. The NP serves in a semi-autonomous role as a medical provider and the clinic team consists of a physician, two nurse practitioners, an LVN, a medical assistant, mental health practitioners and clerical staff.</p>
<p>Qualifications include possession of a CA RN license and Nurse Practitioner certification in primary health care for patients of all ages, as well as fluent language skills in both English and Spanish.</p>
