<p>This is a message from one NAHN member to another.  It’s time to make major decisions about whether or not, we as members of NAHN, are going to play a major role in this coming election.  Voting is a major responsibility.  If we don’t vote then all we can do is blame ourselves.  If you don’t vote, don’t gripe about what is happening around you.  You gave up that right when you did not take the time to vote.</p>
<p>Two years ago, in NAHN, we had the opportunity to elect someone of our own choice.  This may not have occurred because we did not submit our vote. We had less than 10 percent of our membership vote. This occurs because we continually have some of the lowest turnout in voting. Unfortunately we do not take the time to vote.</p>
<p>By June 21st  we have the opportunity again to take charge of who becomes  leaders in our association..  “Are we going to do that?”  We all know that the only way we will do that is by voting.  Our voice counts only if we mark the ballot and send it back.  We have candidates who have shown conviction and commitment to NAHN by taking the risk to run for offices.  Some have shown so much commitment that they are running again. I say let us as NAHN members show that we are responsible, concerned members by voting and making an impact in this year’s election. Tomen el tiempo para votar.</p>

			<content:encoded><![CDATA[<p>       Having arrived to Port au Prince<br />
approximately 10 days after the quake through<br />
Santo Domingo by bus, I was never prepared<br />
for the lack of organization found on the<br />
ground. Walking to the airport looked like an<br />
old Vietnam War movie where helicopters and<br />
military planes landed and took off moments<br />
apart. Meanwhile thousands of people formed<br />
throngs in the streets attempting to enter both<br />
the airport and the industrial center where the<br />
Red Cross was setup.  Entrance was fast and<br />
easy for those with foreign passports, quite<br />
challenging for what the military called the<br />
native population.<br />
       Once inside the airport, I immediately<br />
found the University of Miami tent hospital and<br />
got straight to work.  Even MacGyver would<br />
sweat in this place. No electricity, no<br />
supplemental oxygen, very few medications<br />
initially, no dialysis, very limited equipment in<br />
every way. The first few days I saw many<br />
trauma patients just having found medical<br />
care, by the end of my two weeks the<br />
transition was being made to more chronic<br />
care conditions. What did not change was the<br />
lack of nurses. Doctors, doctors everywhere,<br />
but not enough nurses for medication<br />
administration, non-existent vital sign<br />
documentation, severe lack of specialized<br />
nurses in the areas of critical care/emergency<br />
/community health.  Many of the surgeons<br />
brought their own nurses from overseas, but<br />
there were more than enough hands in the<br />
OR&#8217;s.<br />
        Most of the critical patients were flown<br />
out to Miami for care that could not be<br />
provided in the capital, including our pediatric<br />
patients with tetanus and C-spine fracture<br />
patients.  Many more received amputations,<br />
often of multiple limbs where we discharged<br />
them home with directions to follow up at a<br />
clinic we hope one day would exist and<br />
without the crutches or wheelchairs they<br />
needed, but we did not have.<br />
       Even having lost homes, family, jobs, and<br />
health, the people celebrated hope at every<br />
opportunity. My first night one of my patients<br />
delivered her first child, a boy, to the applause<br />
and singing of the hundreds of patients in the<br />
tent.  After having helped deliver babies in<br />
elevators, cars, sidewalks, and parking lots<br />
here in the states, I still remain overwhelmingly<br />
impressed with the delivery of that child,<br />
Jefferson.<br />
       Many people ask if the experience was<br />
traumatic for me, seeing the amputations and<br />
injuries, dead bodies, etc.  Honestly, it was<br />
not.  No worse than what is found in Brasil, El<br />
Salvador, or many of the urban areas high in<br />
trauma in the U.S. The acuity was not<br />
impressive so much as the quantity of sick and<br />
injured was.  The patients in the hospitals I<br />
attended to seemed to have at least some<br />
small hope in the way they spoke to you.<br />
       In the refugee camps it was often a<br />
different story. Fighting broke out frequently in<br />
front of us over water or food (though all were<br />
orderly about receiving medical attention). As<br />
each day went on the situation became more<br />
desperate as the few strong men hoarded the<br />
hastily-dropped food and water supplies,<br />
leaving most of the refugees to search<br />
elsewhere.  Combine this with pre-earthquake<br />
stockpiles having run out and the military<br />
halting many supply drops until the security<br />
could first be established and you get a people<br />
who do not know how or if they will survive.<br />
       There was of course a minority who lost<br />
less in the earthquake and those fortunate<br />
enough to be sent to the Dominican Republic<br />
or Miami for medical care.  The stories of thos<br />
fortunate few I only glimpsed briefly at the<br />
national hospital in Santo Domingo.<br />
       The international community did an<br />
impressive job at coordinating<br />
multi-disciplinary care at the General Hospital<br />
in Port au Prince, as the Swiss ran the<br />
pediatric units, the Norweigans ran the skin<br />
graft unit, and the French an OR for<br />
amputations and sterile dressing changes<br />
QOD.  The Americans ran the ER, several<br />
OR&#8217;s, the pre and post-op units, as well as the<br />
critical care unit.  During my time there this<br />
was overseen by the International Medical<br />
Corp, and I was told Johns Hopkins was<br />
coming in to take over shortly thereafter.<br />
       Aside from the care we provided, I would<br />
have to say one of my favorite things about<br />
the teams I worked with was the humility.<br />
Everyone, including doctors, were on a first<br />
name basis and whether you were a<br />
translator, transporter, CNA, RN, or<br />
self-proclaimed deity, you looked all in the eye<br />
with the same level of respect and<br />
appreciation.  VIVE LA HAITI!!!!</p>

		<title>The Pressure is High and the Smile Continues</title>

			<content:encoded><![CDATA[<p>Texas celebrates National Hispanic Heritage Month  from September 15 through October 15. My employer, Del Mar College, is fortunate in that we have an Hispanic minded organization Texas Association of Chicanos in Higher Education (TACHE) which puts together different functions for the college staff and community.  I am a member and was fortunate to serve on the committee this year.</p>
<p>As part of our NAHN chapter community contribution, several members did Blood Pressure checks at both campuses on three different days.  On the first day, September 15<sup>th</sup> at around 1030, an employee came in to get her reading.  I took the blood pressure, paused, took a deep breath, and then took it again. Here was this black female, about 170 pounds, 5’5” tall, about 55 years old (maybe younger) with a reading of 180/110. On the second reading (I follow what I teach) I got about the same. With my pulse slightly elevated, and I’m sure my voice up a few octaves,  I asked her  in my most professional manner: ‘ Have you ever had high blood pressure?’.  She replied that she had been told she had high blood pressure and had been given “some pills” to take. She took them a few days then promptly quit taking them and did not return for monitoring.</p>
<p>Of course, many things are going through my mind as the Nursing Process is evolving through my brain. <strong><em>THINK</em></strong>! I keep telling it, but it got frozen in time I guess.  I looked at her and told her what the reading was, expecting to have a semi-hysterical woman on my hands. She gave me the prettiest smile and said something like ‘are you for real’. Then she proceeds to tell me it has been almost that high before. I got some history and she said she had some  family with hypertension, some history of heart attacks.  I quickly went into my teaching mode, etc. risk factors, etc, prevention, etc. need for medication, etc. All this while she is smiling at me.  She had the nicest smile.</p>
<p>I offered to call a family member to take her to the doctor since I did not want her driving. She promptly declined, said she would go to her office and call her doctor. I made her promise (almost to the point of getting it in blood!). I even offered to call the physician myself, I was going to plead (beg or threaten) with the physician to see this woman today! After all am I not a patient advocate? She left the room where were checking blood pressures, escorted by 2 co-workers who promised to send her home.  She gave me the warmest, biggest smile.</p>
<p>At 4:45pm, I called her department to ask if she had left to see her physician. Guess who is on the other end of the line? You guessed it – then I really needed someone to check <em>my </em>pressure.  She said was just going to call. I reminded her that most offices close at 5:00 pm and strongly urged her to call. Again, I offered to call for her. No, she would do it herself. I began to wonder if perhaps there was a streak of stubbornness here.</p>
<p>Jump forward from that date to September 28<sup>th</sup> when we are back at the West Campus again doing blood pressure checks. My beautiful smiling friend walks in and gives me an even bigger smile and hug. She tells me she went to the doctor about 5 or 6 days after I checked her pressure. While in the doctor’s office, her pressure was 220/120. She then proceeded to have a cardiac arrest in the doctor’s office! She had a few days stay in ICU and telemetry. Her doctor told her that if she had not been in his office, she would not have survived. He initiated CPR and fortunately for my smiling friend, his office is right next door to a hospital.</p>
<p>She tells me she is on medication and plans to take it as ordered. I told her I was so glad to see her and grateful that she went to her doctor. She said she now understands the dangers involved with her high blood pressure.</p>
<p>I pray that she does. I had shared the story with a coworker and found out this same person had had several episodes where her pressure had been high yet had not sought medical attention. Perhaps this time she will. Her smile is too beautiful to not have around.</p>
<p>Those of us who did the blood pressure checks had a really great time providing education to the students, faculty, and staff who stopped by. We even got a write up in the school newspaper, picture and all! We are aware of what hypertension does and want to expand this to our Hispanic neighborhoods. I personally feel that perhaps I had a small part in contributing to this lady’s continued stay here. Perhaps we will soon grow in membership to provide this and more to our community.</p>
